Instructions for using the Electronic Health Book

Note, the phone number you use to register the Electronic Health Book will match the phone number registered with the vaccination unit. Therefore, the Health Authority will use this phone number to receive any injection reaction notices from you.

Step 1:

First of all, download the Electronic Health Book application according to the link below and install it as usual.

Step 2:

At the application interface, click Subscribe button to create an account using the Electronic Health Book. Next to us Fill in all personal information, the account is required as shown.

Step 3:

At this point you will received a confirmation code sent to the phone number số to fill in the interface of the application. So you have successfully registered an account. Return to login to the application to use. If the interface for additional information is displayed, you can click Back to add it later.

Step 4:

At the main interface of the application, click section Post-injection reactions to fill in information. Once you have completed your COVID-19 vaccination, the injection information is automatically filled in due to synchronization with the National Immunization System information used in health facilities.

Below if there is a reaction after the injection, click Yes and then select the reactions you encounter. The symptoms that you have experienced need to be listed accurately and in detail. Then click the Save button to save it as a survey.
